Aims
In recent years, it has become apparent that there are cases where kidney function declines rapidly from the stage of microalbuminuria without high-grade proteinuria (rapid decliner), which is different from the classic course of diabetes, and this has become a problem.In addition, a new concept of diabetic kidney disease has been proposed as a more comprehensive disease concept for chronic kidney disease associated with diabetes.The factors and molecular mechanisms that determine the course of diabetic nephropathy, whether it is the classic diabetic nephropathy or the rapid decliner, are unknown.In order to develop effective diagnostic and therapeutic methods for individual cases and conditions of diabetic nephropathy patients, it is necessary to elucidate the molecular mechanisms.

Participants/materials
Male patients aged 20 to 75 years with a history of diabetes for more than 10 years, an eGFR of less than 60 mL/min/1.73m² at their most recent outpatient visit, who were taking a RAS inhibitor but not a SGLT2 inhibitor and were scheduled to undergo a renal biopsy.
