SUMMARY

Aims: Liver diseases, including chronic hepatitis, cirrhosis, and liver cancer, are prevalent in Japan, which are often caused by hepatitis viral infections. The pathogenesis of hepatitis and the selection of antiviral therapies varies depending on the genotypes or sequences of the viruses. Genes and gene expression levels of the host are also involved in the pathogenesis. Recently, along with genomics and transcriptomics, proteomics and metagenomics are also available which have revealed that gut flora is also associated with the pathogenesis and therapeutic efficacy of drugs in chronic hepatitis. The aim of this study is to establish a comprehensive genetic diagnosis system for chronic liver diseases by investigating genes and proteins associated with chronic liver diseases from both the viral and host side.

Methods: 16S metagenomic analysis was performed on stool DNA samples collected from 27 patients who experienced liver biopsy according to the following criteria or clinically diagnosed severe non-B non-C cirrhosis at the University of Tokyo Hospital. After excluding patients who had never been diagnosed with diabetes (6 cases) and patients who were categorized to Matteoni classification class 2 or 1 (1 case), 20 patients with diabetes who were diagnosed NASH as Matteoni classification class 4 or clinically diagnosed liver cirrhosis were analyzed. The 16S metagenomic data from 5 patients who were under insulin treatment were compared with those of the remaining 15 patients.
